Referrals

In order to help you we need to ask for and store information about you. The law states that we must get your consent to do this. By signing this form you will provide your written consent for us to hold your details securely on our electronic database and, where necessary, in paper files. Everything you tell us will be treated confidentially and your data will be subject to the Data Protection Act 1998 and General Data Protection Regulations (GDPR) 2018.

• We will not share your personal information with anyone without your permission except in circumstances where the law permits us to do so; for example if you disclose that you or someone that you know is at risk of harm or if a court requests information from us.
• Information will only be seen within our organisation by those who need access to it.
• In order to provide you with a full support service your information may be shared with other organisations for your benefit and in order to access other services for you.
• When your file with us is closed, your information will be safely stored with us for a period of 6 years, as the law stipulates and in line with best practice. The information will then be securely destroyed.
• Your information may be used for statistical reporting but all data will be anonymised.
• You have a right to ask for access to any information that we hold about you as per GDPR.
• We do use a third party to maintain our IT systems who operate in a secure way on behalf of our organisation and who also adhere to the regulations above.
• You have the right to withdraw your consent at any time, subject to the stipulations above.
