• My Time Grant

  We are able to administer the My Time grant which allows carers to apply for £100 per calendar year to spend on a break from their caring role. Applications can be placed by an individual or by a professional working with them. All forms need verifying by a professional who can confirm that the carer meet the criteria. • Mid Yorkshire Hospitals Trust

  We have Carer Support and Liaison Workers working within the Mid Yorkshire Hospital Trust, based at Pinderfields Hospital. The Carer Support Workers work in partnership with the staff on the wards (nurses, PT. OT etc.) and the hospital based Social Workers.  They can attend discharge meetings and other key meetings to provide the link between the patients family and the medical team.

• GP Practices – Pink cards

  For those carers not registered with Carers Wakefield & District (CWD), we have an established referral system for GP practices – The Pink Card System. The majority of GP practices across the Wakefield and five towns districts use this system. GP staff can, with consent, complete a CWD pink card with the carers details which can be returned to us either electronically or via post. The carer can then be coded onto your system as a carer to access support.

• GP Practice Training

  We are able to offer bespoke carers awareness training for all practice staff including:

  • How to identify carers,
  • How to refer to us via the Pink Card System
  • The roles carers have in looking after their loved ones
  • Carers Wakefield and District offer to all carers.
  • GP Carer Quality Markers
  • Carer Champion Training

  We can run Carer Clinics – this is where your patients can see one of our support workers at your practice. This can be a drop-in clinic or booked appointments.

• Events/Presentations

  Our Staff will attend any event that will raise awareness of carers and the work that we do, either organised by a GP Practice or by other professional and voluntary organisation, as well as give presentations to highlight the role of the carers and the way in which our service can support them.
